AHPRA's Two-Tier Justice: Common Sense Punished, Ideologues Protected
Australia's peak medical regulator has just proven whose side it's on. Doctors who question the "gender-affirming" treatment of children are hunted down with anonymous complaints and emergency bans. Doctors who champion that same model, even after a judge found their expertise was partisan advocacy, are protected.
A Complaint That Surfaces Right on Cue
Professor Gary Geelhoed spent a career at the top of Australian paediatrics: former Chief Medical Officer of Western Australia, 22 years running the emergency department at Princess Margaret Hospital for Children, and later he became the executive director for the WA Health Translation Network. He is precisely the kind of senior, credentialed clinician the public trusts.
In January 2026, Professor Geelhoed went on the record in The Australian to say what many paediatricians privately believe: that the gender clinic at Perth Children's Hospital enables the referral of gender-distressed teenage girls to private surgeons for double mastectomies, procedures he does not believe a confused adolescent can meaningfully consent to. He didn't mince words, calling the treatment of gender-distressed minors "the biggest medical scandal of my time in medicine."
This month, he did something the Australian Health Practitioner Regulation Agency (AHPRA) apparently found unforgivable: he publicly challenged the regulator itself over its treatment of Queensland psychiatrist Dr Andrew Amos. Within days, AHPRA surfaced a complaint against Professor Geelhoed that was anonymous, and sat untouched for six months. Nobody had acted on it. Nobody, it seems, cared enough about it until he challenged the regulator in public.
Dr Amos: Punished First, Never Investigated
To understand why Professor Geelhoed's challenge mattered, you need to understand what happened to Dr Amos. Dr Amos is a Queensland psychiatrist and the chair of the Queensland Section of Rural Psychiatry with the Royal Australian and New Zealand College of Psychiatrists (RANZCP). Dr Amos published peer-reviewed critiques of the gender-affirming model of care, arguing that it prevents psychiatrists from properly assessing whether underlying mental illness is driving a young person's gender distress. That is not misinformation; it is a clinical argument, made in the normal channels of medical scholarship.
It didn't matter. In February 2026, acting on four complaints, with at least one being anonymous, AHPRA invoked its "immediate action" power against Dr Amos, the regulator's most extreme tool. It requires only a "reasonable belief" of risk, no completed investigation, and no finding of misconduct. AHPRA used it anyway: Dr Amos was banned from posting about gender medicine on social media and barred from any clinical contact with patients, while a full investigation that might actually establish whether he did anything wrong could run for up to two years. His own professional college, the RANZCP, piled on with a suspension.
No patient was harmed. No finding was made. Instead, a senior clinician was suspended for dissenting against the “approved position” on this controversial issue.

One Rule for Believers, Another for Sceptics
Now compare that to what happened to Dr Michelle Telfer. As chief of medicine at Melbourne's Royal Children's Hospital, and the doctor who authored the 2018 guidelines that spread the gender-affirming model across the country, Dr Telfer is as close to gender medicine royalty as Australia has. Earlier this year, a Family Court judge found that her expert testimony in a custody case had crossed the line from evidence into advocacy, pointedly noting that "advocacy in a court is for lawyers, not witnesses, neither lay nor expert." A complaint followed, alleging she had failed her duty as an objective expert witness.
AHPRA's response? It cleared her with no conditions, no restrictions, just a letter finding she was "meeting accepted standards of practice."
Sit with that for a moment. A psychiatrist who published academic critiques of a treatment model was banned from discussing the subject, on the word of anonymous complainants, with no finding against him. A paediatrician who raised public safety concerns has an old anonymous complaint mysteriously resurrected the moment he publicly dissents. And the doctor a judge criticised for abandoning objectivity in favour of advocacy? Cleared, standards met, move along. If AHPRA were simply enforcing professional conduct without fear or favour, its outcomes would not sort so neatly by ideology.
Why the Watchdog Isn't Neutral
This pattern isn't an accident, and it isn't a coincidence stacked case by case; it's structural. AHPRA takes part in the Australian Workplace Equality Index alongside Pride in Diversity, the workplace arm of trans lobby group ACON. When a complaint was raised about the regulator's closeness to that same lobby group, it landed with the National Health Practitioner Ombudsman — AHPRA's own watchdog — which dismissed it without ever disclosing that it, too, has ties to ACON. Documents cited by The Australian have reportedly shown just how deep that relationship runs.
A regulator that trains alongside an activist lobby group is overseen by a watchdog with its own undisclosed ties to that same lobby group. Is it any surprise then that this department hands down wildly different verdicts depending on which side of the gender debate a doctor sits on? That is not evidence of tough, impartial calls being made. It is evidence of a captured institution protecting its friends, punishing its critics and dressing it up as "public safety."
The Real Cost
Every doctor watching what happened to Dr Amos, Dr Jillian Spencer and now to Professor Geelhoed, knows exactly what awaits if they speak up. Say the wrong thing about a child's medical care, and the regulator meant to protect patients will instead go looking for a reason to shut you up. Say the right thing, and you can testify like an activist and still be found to be "meeting accepted standards."
This should trouble every Australian, not just families already sceptical of paediatric gender medicine. A regulator that punishes dissent and shields its friends isn't protecting children; it's protecting a narrative. The doctors still willing to ask hard questions about irreversible treatments for confused teenagers deserve a regulator that is neutral. Right now, Professor Geelhoed and Dr Amos don't have one.
